- Individual, $40 ($30 tax-deductible), above listed benefits for one member
- Household, $65 ($40 tax-deductible), above listed benefits for two people in a household
- Family, $100 ($80 tax-deductible), above listed benefits for up to five people in a household
- Student, $30 ($20 tax-deductible), above listed benefits for one student member with valid student ID
- Senior, $30 ($20 tax-deductible), above listed benefits for one adult age 62 or older
